Indiana State Police arrested an Otwell man Tuesday night after they say he tried to obtain prescriptions by fraud and forged several medical documents in the process.
On November 2, ISP launched a criminal investigation after receiving a call from Holland Family Medicine that a man by the name of “Daniel Taylor” had come to their office the day before, with a complaint of back pain. After examining him, the doctor wrote two prescriptions: one for Valium and the other for Loratab.
Later that day, the doctor’s office received a call from the Dale Pharmacy. A pharmacy staff member recognized ‘Daniel” to be Tony Taylor, 48, of Otwell when he came to fill the prescription. When pharmacy personnel ran computer checks on Tony they discovered an extensive history that involved multiple visits to different doctors and attempting to fill multiple prescription for narcotic drugs.
